Emergency telemedicine: COD 19 and COD 20 systems
About this good practice
The COD 19 Platform is a telemonitoring system to cope with the high number of people affected by COVID 19 in the wards of Sacco Hospital in Milan. The system was created to follow up on discharged patients:
Creation and development of COD 19. In March 2020, the university commissioned a digitised system to a small IT company. Patient Lists. From the earliest stages of the pandemic, infectivologists provided COD 19 operators with lists of hospital discharged patients. Establishment of the first operative unit. The operators initially started by using phone calls and an Excel sheet with patients' contact details and update information on their health status. Functioning of COD 19. The data collected allowed effective communication of the patients' health status to different stakeholders [infectivologists, general practitioners and Metropolitan health authority].
COD 20 platform acts as a platform for: Telemedicine. Different Hospital Devices for different pathologies. Tele-visit. Remote check-ups and modulate drug therapy. Teleconsultation. Several specialists follow the same patient. Health Tele cooperation. The specialist in the hospital communicates with a caregiver. Functioning of COD 20. The platform allows files to be exchanged between general practitioners and specialists. Functioning of a Tele-visit. The system automatically sends a text message and an email confirming the visit to the patient. The platform also transfers the data and the reports to the patient.

Evidence of success
The implementation of COD 19 and COD 20 has enabled coordinated management of remote medical examinations in safety for both doctors and patients.
Potential for learning or transfer
The lessons learned could be summarized as follows:
• Developing an innovative digital platform enabled to track remotely the progress of the disease for non-serious cases.
• Allowing effective communication on the patients' health status among different stakeholders, such as specialists, infectivologists and Metropolitan health authority.
• Developing a new prototype of telemedicine platform dedicated to other diseases, with the possibility of monitoring, cooperating between specialists and caregiver and visiting from a distance.
• Expanding the new tools to other territories in the region.
