About this good practice
The project was considered in the domain B social participation, because it aims to support the self-determined living in their living environment’s for person with disabilities and special needs.
District managers try to improve the neighbourhood cooperation, support institutional cooperation to develop better supply structures and consult in urban planning development processes, whereas neighbourhood guides help individuals to find the needed mix of support in their living environment. In addition, the project coordinates their activities with other projects that are working in the same field of activities.
The Evangelic Foundation Alsterdorf has long-term experience in different fields of social work and is well integrated in Hamburg’s social secure system. Out of this experience Alsterdorf developed the project idea Q8.
The project brings together different stakeholders like companies and out of the public sector to work together on the project goals. The key role in this project has the “district guides” who helps persons with a need for assistance, to stay self-determined in their living environment.

- Development of an inclusive health promotion programme for the employees of day care centres.
- Development of a series of workshops to strengthen the health literacy of people with disabilities in the housing groups of the integration assistance on the topics "doctor's visit" and "hospital stay".

Potential for learning or transfer
The project is a good example how persons with complex disabilities are enabled self-determined in their living environment. The important role of the “district guides” in this project, helps to bring the support to the persons who need it. There for it is a good example how to work with a relatively ‘low-threshold' on the improvement of living condition in the districts.
The project was considered under the Domain B social integration, because it has a very strong link to disabilities and person who need care, and how they can life self-determined in their living environment. It has not such a strong technical approach like other projects from Hamburg. It is a lot about bringing existing structures and services together, to make person cantered individualized solutions possible.
