EKOSMART – SMART INTEGRATED HEALTHCARE AND LONGTERM CARE SYSTEM

About this good practice
Ekosmart is a large research project in Slovenija. One of its field of research (smart integrated healthcare and longterm care system) aims to enable elderly population, people with chronic diseases, dementia or other health conditions, longer, more active and safer conditions to remain living independent in their own homes, resulted in a social care service based on IoT (Internet of Things) and person-centered technologies, available 24 hours a day. The technology supporting the service detects a need for intervention if a user is due to falling or other difficult health condition not able to make an urgent call.
Ekosmart is developing approaches and prototypes to ensure the basic conditions for effective transformation to integrated healthcare and home care system. It provides: integration of different levels of healthcare, effective and secure exchange of information among different stakeholders of the healthcare system on the national level (national registers, accounting system, big data analysis) and basic conditions for development and sustainability of the healthcare and social system. The main goals are: development of a model of integrated healthcare provision and the establishment of related infrastructure, development of a systemic foundations for extending the model of integrated healthcare provision, raise the quality of life and safety of chronic patients and extended care, safe use of medicines and reduced number of referrals to clinical pharmacologists.

Resources needed
Total project budget: 8.661.820,50 €.
The budget is distributed between different categories:
Personnel cost declared as a unit cost 76,64%,
Costs of sub-contracting 4,20%,
Indirect costs declared using a flat rate 19,16%.

Evidence of success
300 users already use the service and by the end of 2019 around 3000 users are expected. Due to project activities the new legislation on long-term care was amended that state should co-finance such services for at least 37.500 users in Slovenia which will boost the acceptance of the service for patients with diabetes type 2, congestive heart failure, arterial hypertension, asthma and COPD (Chronic Obstructive Pulmonary Disease) and other difficult health conditions.
Potential for learning or transfer
The project is a home care project by design. Transferability to other countries/regions could be performed as a transfer of tasks this project encounters in order to integrate all helixes in the R&D phase and make a market ready customer services. Each country has specifics in the legislation, user demands (in details), available income, social security system etc, however general demands and demographics is equal.
This good practice brings, for each public authority, an example how to establish relevant and well-functioning ecosystem which is a base for designing solution according to the local needs. It is mandatory to include all 4 helixes and connect health care and social system in the country because the most difficult work is not technology but well-functioning ecosystem.
