GWALENN portal for health & care professionals
About this good practice
GWALENN is made up of 3 modules:
‐ Orientation desk: it is the entry point where a professional
can alert (detection of the need), evaluate and ask for support for
the orientation of the patient ;
‐ Coordination file: shared patient individual file, with
administrative data, list of involved health and care professionals,
coordination file, follow‐up forms;
‐ Organisation file (own file for each organisation member of
the coordination action who can extract its own activity report for
example).
The GWALENN portal is dedicated to health and care insitutions, but
a new function is being developed for health and care professionals
in mobility (from a mobile phone): Mobil’eTY by Globule enables
secure communication (to avoid the use of texts, whatsapp, etc.)
and gives access to the orientation desk and to a summary of the
coordination file.

Expert opinion
The rise in chronic diseases, loss of autonomy and the growing number of elderly and disabled people mean that care and services need to be developed and diversified, but also that interventions need to be better coordinated. The Gwalenn online platform has been designed to to better coordinate different components of care for each patient in this context. It is specifically designed for complex cases, for which individual healthcare professionals can spend much time obtaining relevant expert opinions, identifying available resources (e.g. hospital beds) and keeping track of different components of each patient’s care. The time saved on these tasks can be made available for medical activities. This is particularly important in a context of healthcare professional shortage. The design of the Gwalenn platform is the outcome of a participative process. Its specifications were co-constructed by a wide range of actors over two years. This made it possible to build on an in-depth understanding of user needs in the design of the platform. The practice is therefore a good example of how e-health technologies can be used to strengthen links between healthcare professionals and improve the quality and cost-efficiency of healthcare provision, by applying a well-designed co-construction approach.

Evidence of success
The project started in 2017.
In 2019, nearly 40 health and care coordination organisations are using it, with more than 200 users.
In March 2019, the ARS has launched a second call for interest to
identify additional organisations and deploy GWALENN further in
the region. The tool is still under development within the Brittany Region.
Potential for learning or transfer
The tool was improved by the supplier in link with the professionals requests thanks to the GCS e-Santé.
1. The tool makes other French Regions want (Grand Est, Guadeloupe).
2. The tool could also be extended to social services, departments and agents.
It has a real transferable dimension.
