
HEALTH NETWORK LAS FUENTES

About this good practice
Health Network in Las Fuentes was born in 2018 from an initiative promoted by the City Council of Zaragoza and the Government of Aragon with the aim of promoting collaborative work and making visible the community health actions that are carried out, promoting coordination between the resources that work in the neighbourhood and boosting health assets, encouraging the participation of citizens and social groups in the improvement of health and quality of life.
All this taking into account the fact that social factors in health are increasingly relevant, as the environment in which we live is a determining factor in our health and favours or deteriorates it.
The neighbourhood in which we are acting has very defined and homogeneous characteristics, arising from the rural exodus of the industrialisation of the sixties of the 20th century, it currently has:
- high rates of ageing and over-ageing.
- low economic level.
- low educational level.
- high percentage of people with chronic pathologies.
- Aging housing stock, including areas of substandard housing.
- High percentage of foreign population.
- Low level of associations.
In 2019, following the technical diagnosis of Salud en Red en las Fuentes, the roadmap to be worked on in the following years included tackling health determinants, taking into account life transitions and healthy environments.
Resources needed
The financing comes from the Administration's own funds and from resources provided by the different entities, but the key is human resources GRUPO MOTOR: health professionals, Municipal Social Services Centre, Municipal Centre for the Elderly, schools, high schools and social entities.
Evidence of success
The evidence of success is that since 2022 a monthly coordination meeting has been held for all the resources that are directly or indirectly related to health. Following the social diagnosis based on social determinants of health, each year the driving group establishes lines of action from the beginning to their evaluation, always with the participation of citizens and with a community vision.
Potential for learning or transfer
We believe that it has a great potential for learning or knowledge transfer since it has created an enriching and intense working space for professionals from different disciplines and institutions and has promoted the participation of the neighbourhood of a neighbourhood in relation to health and community wellbeing.
In addition, it has improved the wellbeing and meeting and relationship spaces demanded by the neighbourhood to address and promote the importance of health assets.
