About this good practice
The main objective of the project was to demonstrate how ICT can support the diagnosis and treatment of patients with COPD, facilitate access to specialist consultations and the flow of information between patients, HCPs and specialists.
The proposed telecare model used a web-based platform to coordinate patient activities and a website as an educational tool for patients.
Path I treatment:
Facilitating access to specialist consultations and information for patients and better information flow between primary care physicians and specialists
Path II treatment:
Video consultations with specialists and support from assistants provided to patients after severe COPD exacerbations (or at risk of it), preventing further exacerbations (including rehospitalization) and
improving self-management with chronic disease

-510 patients who will be referred for teleconsultation to UCK - (Path I)
-70 patients with severe exacerbations or frequent exacerbations of COPD (Path II treatment

Evidence of success
The project provided support (diagnosis, treatment plan, medical consultation) to 580 patients with COPD, including 70 patients following a severe exacerbation of COPD.
The training and education of patients in the project contributed to their greater involvement in the treatment process. The telemedicine solutions used in the project helped reduce the burden on patients of travelling to medical facilities and facilitated access to medical care.
Potential for learning or transfer
One of the key issues in the project was the organisation of a properly functioning COPD telecare system (organisation, technology, logistics, procedures, personnel) based on multi-centre collaboration. Through the implementation of this project, mechanisms have been developed, much of which will be implemented in the routine care of COPD patients in the participating healthcare entities after the end of the project. The implementation of this project has also confirmed the need for a strong involvement of both the medical team and nursing and support staff, such as medical coordinators, in this type of care. The collaborative model developed in the project and tested in a real-life setting can also be implemented by other healthcare providers involved in the diagnosis and treatment of patients with COPD or provide valuable inspiration for them to develop their own model.
