REHAB_DK
About this good practice
In collaboration with the municipalities, Syddansk Sundhedsinnovation has developed a rehabilitation portal that meets wishes in the field of rehabilitation. The result is REHAB|dk.
REHAB|dk is a digital rehabilitation solution that raises the quality of home-based training and empower citizens to take responsibility for their rehabilitation.
The solution consists of a web portal (www.genoptræn.dk), where healthcare professionals (HCPs) can generate programs and monitor progress of each user. An app for citizens (iOS and Android), where they can follow their progress, rate intensity of pain and see all the exercises demonstrated by a physiotherapist.
New videos are recorded based on the needs of municipalities and hospitals and made in close collaboration with physio- and occupational therapists to ensure high quality.
The citizen’s personal therapist designs individual exercise programs at the first consultation.
REHAB|dk is not a substitute for personal contact, but a supporting tool for both patient and therapist in the rehabilitation process
Resources needed
REHAB.dks front and back-end = 300,000 euros.
The content = 400 euros per video.
Implementation includes a kickoff and staff training, and futher development requires specific ressources.
Operation of REHAB-dk = 161,300 euro per year (split with 50% from the municipalities & 50% from the region)
Evidence of success
REHAB|dk works with a high degree of user involvement, and there are currently more than 750 video exercises, all recorded based on needs from all sectors, and exercises for a wide range of patients.
In 2019, the Committee for Health IT decided to implement REHAB|dk in hospitals in Region Southern Denmark, to improve coherence in the patient process.
The information is provided based on patient-reported information (PRO), and the citizen will experience increased coherence in the process
Potential for learning or transfer
Clinicians might face challenges in accessing patient data or collaborating with hospital staff if the system is not fully embraced by hospital management.
It highlights the importance of providing clinicians with access to up-to-date research, best practices, and treatment guidelines. Clinicians using the system may benefit from features that facilitate knowledge sharing, collaboration with experts, and access to evidence-based resources to improve patient care
Within a remote healthcare system, features that support communication and collaboration among clinicians, hospitals, municipalities, and other stakeholders are essential.
The system can facilitate this goal by providing tools and resources for clinicians to develop and implement individualized care plans, track patient progress remotely, and communicate effectively with patients and their caregivers. Features such as telemedicine consultations, remote monitoring devices, and patient education materials can support clinicians
