About this good practice
Porto4Ageing competency centre is a two-starred Reference Site and Partner Plus of the European Innovation Partnership and Healthy Ageing of the European Commission, bringing together over 90 organisations, the large majority established within Porto Metropolitan Area, in the Northern Region of Portugal. The partnership involves different stakeholders (regional governments, health and care providers, academia and research, industry and civil society), driving structural changes far beyond the scope any organization could achieve on its own.
This is an integrated care model applied on a daily basis, from primary to tertiary care, through all the health care providers: it covers the majority of care required in the older population, from mental health (psychology and psychiatry), to cardiology, cardiovascular and palliative care, neurology, oncology, pulmonology, amongst others. It provides specialized training to health and other professionals and deals with the remote monitorization of chronic conditions at home, and participates in research projects approaching this issue, to name a few activities.
The main stakeholders are the regional governments, health and care providers, academia and research, industry and civil society, and the main beneficiaries are the elder population.

Resources needed
The implementation costs to set up the good practice required about 60.000€; the remaining costs are variable. On what comes to human resources, the projects counts with about 3 full-time employees.
Evidence of success
Porto4Ageing is a two-starred Reference Site and Partner Plus of the European Innovation Partnership and Healthy Ageing (EIP-AHA) of the European Commission; it brings almost 100 organizations together, the large majority within the Norte Region of Portugal, and this number is increasing due to the interest of this consortium for regional organizations; the number of projects financed proposed by the consortium increases each year; finally, it's a member of some European and national networks.
Potential for learning or transfer
The project works within the EIP comprehensive and designated areas for intervention (adherence to health plans, prevention of falls, prevention of fragility and functional decline, integrated care, solutions for independent living and friendly environment to the elderly, among others).
This networking can be established amongst other analog institutions in Europe - as long as the the Quadruple Helix-Based Innovation Model of Reference is used as a model, there’s the possibility to transfer this network innovation model to other contexts.
